0. Fix open quotations so they work on unindended paragraphs

1. Remove underscores from _long_style_italics_

2. More difficult will be support for boldface:

	*bold* *words* and *bold phrases*

	***bold*** ***words*** and ***bold phrases***

but not:

	* bulleted text

	wildchar chars like *.txt which may happen in a row:
	*.txt,*.text, etc.

	footnotes.*

	*scratch* in context of Emacs

(perhaps only bold numerals and uppercase words?)

3. Boxes and borders using characters including asterisks and
underlines.
